Coordinated dual-hormone artificial pancreas with parallel control structure

Concomitant glucagon delivery in a dual-hormone artificial pancreas system might overcome the limitations of insulin-only systems, potentially leading to improved hypoglycemia mitigation. However, current dual-hormone systems have not shown clearly its superiority. This work presents a novel dual-hormone system based on a parallel control structure with intrinsic coordination among insulin and glucagon delivery. The proposed coordinated controller (CC) was compared with its non-coordinated counterpart (NCC) based on independent loops for the insulin and glucagon controllers, as done in current systems. To evaluate the benefits of coordination, CC and NCC shared the same insulin controller, differing solely on how glucagon is administered. An in-silico study using the UVA-Padova simulator, extended with variability sources, was performed. Three scenarios were considered, comprising meals, snacks and exercise. CC achieved superior performance with better values of times in ranges. Moreover, CC delivered lower glucagon doses than the two NCC tuning alternatives considered. (C) 2019 Elsevier Ltd.
